Home Health Registered Nurse RN

Job Type: FullTime / PartTime / Per Diem

Job Summary: We are seeking a compassionate and dedicated Hospice Home Health Nurse to join our team. The ideal candidate will provide exceptional care to patients with terminal illnesses and support their families in a home setting. This role involves managing patient care plans administering medications and offering emotional support.

Key Responsibilities:

• Deliver highquality compassionate care to patients in their homes.
• Assess and evaluate patient conditions and progress.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Administer medications and treatments as prescribed by physicians.
• Provide education and support to patients and their families regarding endoflife care.
• Document patient progress and maintain accurate medical records.
• Coordinate with other healthcare professionals to ensure comprehensive care.
• Offer emotional support and counseling to patients and families.

Qualifications:

• Registered Nurse (RN) license required.
• Previous experience in hospice or palliative care preferred.
• Strong clinical assessment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate patientcentered approach to care.
